After booking, please be sure to complete Intake that is emailed to you at least 24 hours prior to your appointment and review the "How to Prepare For Your Visit" tab.

The fees listed serve as a deposit for your appointment and are due on the day of the appointment. If we are Out of Network with your insurer, you may still be eligible for insurance reimbursement under the Affordable Care Act. We will either submit a claim to your insurance company on your behalf or provide you with a superbill to submit to your insurer. If your insurance company pays on a claim we file for you, you will be promptly refunded.
