Aetna and Humana Members: We suggest you confirm your lactation benefits with your plan prior to your visit. Your insurer may tell you that you are covered 100%, but it is possible that some insurance plans may assign you a ‘Patient Responsibility’ fee which will be charged to the card you have on file.
Out of Network: The fees listed serve as a deposit for your appointment and are due on the day of the appointment. If we are Out of Network with your insurer, you may still be eligible for insurance reimbursement under the Affordable Care Act. We will provide you with a superbill to submit to your insurer. If your insurance company pays on a claim we file for you, you will be promptly refunded.
